Transfer Student Registration & Enrollment Information

To be considered for a transfer, families must complete the standard registration process. Admissions are subject to grade-level seat availability and programmatic capacity.

1. Submit a Transfer Inquiry: Fill out this form to submit your transfer inquiry. Please note that transfers are only accepted based on availability in grade level. Students will be asked to submit the following:
• 9th Grade Transfers:
• A copy of your 8th Grade Diploma
• A copy of your final 8th Grade Report Card
• 2 copies of proof of address (see list of acceptable proofs here)
• IEP/504 (if applicable)
• 10th – 12th Grade Transfers:
• Most current transcripts
• 2 proofs of address (see list of acceptable proofs here)
• IEP/504 (if applicable)
2. Schedule an Enrollment Appointment: Once paperwork is reviewed and your student is accepted, a registration form will be sent to you for completion and school staff will schedule an appointment to finalize enrollment.

• Please bring the following original documents to your enrollment appointment. Copies will be made for our records.
1. Proof of Graduation/Promotion & Academic Records:
1. Official transcript from the previous high school (including current year schedule and withdrawal grades).
2. Most recent report card.
3. Standardized test scores (if applicable).
2. Proof of Identity & Age:
1. Student’s original birth certificate.
3. Proof of Residency:
1. Two (2) current forms of proof showing the parent/guardian’s name and address (e.g., current utility bill, lease agreement, deed, or driver's license/state ID).
4. Health & Immunization Records:
1. Up-to-date State of Illinois Certificate of Child Health Examination.
2. Proof of current immunizations.
3. Dental/Eye exam forms (if applicable for the grade level).
5. Special Services Records (If Applicable):
1. Current Individualized Education Program (IEP) or 504 Plan.
2. English Language Proficiency scores (ACCESS testing records).
6. All Enrollment CPS Forms (listed below)
