As we continue to operate remotely in an effort to ensure the health and safety of our students and staff, we are offering families two options for enrollment.
- Downloadable forms are on the Kelly website https://www.kellycollegeprep.org
- Forms and documentation may be emailed to Ms. Ayala email@example.com or Ms. Ye firstname.lastname@example.org. Original documents must be submitted once school opens.
- Pick up packet: A packet with forms may be picked up Monday-Friday at school from 9am-1pm through door #7 (Mozart entrance)
- Drop off packet: Forms and original documentation can be dropped off Monday-Friday from 9am-1pm. Documents will be returned once the building opens.
- Parent/Guardian valid ID
- 2 proof of address (Utility Bill, Lease, cable/satellite bill, etc.)
- Birth Certificate
- Transcripts from the previous school mandatory (H.S. Transfer Students Grades 10th-12th)
- Report Card & Diploma (Incoming Freshman only)
- Most updated shot records & physical
